Sacred Heart Hospice Services

 

Our Hospice Team consists of Skilled Nurses, Home Health Aides, Physicians, Pharmacists, Speech Therapists, Chaplains, Counselors, Social Workers, Dietitians and Volunteer Services. Using and interdisciplinary team approach of Hospice personnel, Sacred Heart Hospice provides palliative care in the patient's home, short-term inpatient care, mobilization, coordination of ancillary services and bereavement support. Hospice staff and volunteers offer a specialized knowledge of medical care, including pain management. in addition to the services provided by the hospice team, medication, medical equipment and supplies are provided to enhance the patient's comfort

Sacred Heart Hospice Care Team

Nurses - Each patient is assigned a primary nurse who will educate the patient and family about the disease process as well as symptom management. Nurses will visit as often as needed to assist with any problem that may arise, and will be available to answer questions on the phone

​

A nurse is available 24-hours a day. 7-days a week.

Home Health Aides - Aides provide assistance with personal care, hygiene, and activities of daily living

​

Social Workers - Social workers provide counseling and emotional support for the patient, family and their caregivers. The Social worker is available as a consultant for individual or family emotional issues or to assist in addressing care giving and end of life issues such as Durable Power of Attorney for Health Care and funeral arrangements

Volunteers - Trained volunteers provide companionship, relief and support to patients, families and caregivers.

​

Bereavement Counselor - The Bereavement Counselors maintain regular contact with the family for 12 months or as needed. Bereavement services, like all other hospice services, are matched to the special needs of each family

​

Spiritual Counselor - Spiritual and personal counseling is provided in accordance with the family's wishes and religious preferences.

​

Additional Team Members - The Hospice Medical Director in conjunction with the patient's attending physician oversees and coordinates all aspects of hospice medical care. If appropriate, the Interdisciplinary Team will provide the services of physical therapists, occupational therapists, a dietician or other appropriate health care providers.
